Enrolment Form

Welcome! To enrol your child for music lessons starting in Term 1 2025, please fill out this enrolment form. Note that a separate form must be filled out for each child and/or each instrument. Please double-check that your child is old enough for the class you've selected (or turns old enough before the end of Term 1).

The subscription fee is $150 per enrolment for the full year. Get a $10 'early bird' discount if you enrol before November 30.

Please disclose anything that may affect the student's ability to participate in a group setting. (If none, type "none")

PARENT DETAILS

Please format number with hyphens (e.g. 021-234-5678, or 021-2345-6789).

If providing a mobile number, format as 021-234-5678, or 021-2345-6789.
If providing a landline number, format as 06-345-6789.

CLASS DETAILS

For an extra fee of $85, Saturday Music can hire out instruments for students enrolling in our classes. The following instruments are available for hire: cellos, violins, clarinets, flutes, saxophones, trumpets, french horns and trombones. More information can be found on the instrument hire page.
